Help with translation - 03-24-2010, 02:06 PM

Son-in-Law is a pro basketball player in Japan. Wife and I asked our daughter to send his earnings information so we could help them file income tax here in the US. When he sent the info it was a scan of the document and it was in Japanese and we are unable to translate. Could someone help with this please? See attached jpg. Thanks

給与支払表=Salary payment table
平成21年=2009 10月=OCT 11月=NOV 12月=DEC 
平成22年=2010 1月=JAN 
支払金額=Amount of money of payment

If your son in law got 200000yen on Oct 2009,
Leftside is "平成21年10月分" , rightside is "200000".

Sorry,but I am not interested in Basketball,so I don't know any player.
Maybe your son in law is very tall and good at basketball...That's all I can say about basketball

The left column in the table are dates.

平成 is the name of the Heisei period, and is used to state the year.

月 means month. 1月 is January, 10月 is October, etc.
