Sorry to correct you, but the heterosexual rate of increase in the United States took over before the end of the 1990's. This was in part due to increase prevention eduction efforts in the gay communities and the public shools systems ignoring the issue and refusing to allow such education of students. And it continues to rise because of people who still think they aren't at risk.

It is not limited to 3rd world countries, though the death rates continue to be higher there.
