I don't understand what B mean in his second and third line at all, especially the phrases 専門を決める and オールラウンドにいく. Is this some figurative way of saying? And what does the speaker imply?

There's no figurative speech whatsoever used in phrases 2 and 3 by B. None.

専門を決めないで means "not deciding what area of specialty to pursue". In other words, "Try to like all types of girls, not just nurses."

オールラウンドにいく means "to be an all-round player (with girls)". In other words, "Don't make a fetish of nurses."
