Need Help reading Japanese on online PS2 Game

Okay, I have recently imported this game from japan, and I need help reading this, so can one of you guys please translate it to english for me and tell me what all it is saying, I would really appeciate it!!

I'll translate some very quick and dirty without a care for getting words with their exact equivalent, and also when there was a wall of text i didn't bother because i'm a lazy guy

Creating new user application. Please fill in the below.
1. name (kanji) (full width) (forexample NAME IN KANJI)
2. name (kana) (fullswidth) (for example NAME IN KATAKANA)
3. zip code (half width numerals)
4. "county" (todoufuken) level location (like if you lived in Sagamihara-shi, Kanagawa-ken, youd's elect Kanagawa here, Tokyo would be Tokyo-to I think, etc.)
5. "city" (shikutochouson) level location (numerals also full width) (this is the "Sagamihara-shi" level input or Shinjuku-ku/Shibuya-ku/Nerima-ku/etc. for Tokyo)
6. I guess "block" (chou) level location (these terms for 4-6 just don't exist in English, at least American English) (numerals also full width)
7. name of building/apt/whatever (numerals also full width)
8. phone number for sending you message
for example 031....
Please put a phne number for sending you mesasges in 1.
Please put a different one in 2.

Are you going to request someone translate the entire game? Why did you buy a game that you cannot understand yourself?
