Short questionnaire - Can you please help me?

As I am out of the age range, I just checked the first page of the questionnaire.

Instead, I will make a couple of feedback.

1) In general, total number of the questionnaire pages should be shown in the first page.
2) Most of Japanese teenagers will not start reading English questionnaire. I mean the questionnaire should be in Japanese if you want to get more Japanese respondents. If you expect the respondents to have English ability, English questionnaire will make good filtering.

And I have a question. Why do you expect your questionnaire will be answered by volunteer respondents without any reward? I do not mean money as the reward but some beautiful picture of Netherlands or future telling calculated from the answers.
