Should be a direct train ride from Hiroshima Station to Miyajima-Guchi, then from there you take the ferry to Miyajoma, he shouldn't need to do a transfer

When you take the tram to Miyagima you have to change after a few stops but not too much of a problem. Ask the staff at the hotel and they will help. You can get a Miyagima pass which includes the tram and ferry from memory. Not sure though so better to ask for the name of the pass and then you can get it from the tram conductor.
