Help with translation

Son-in-Law is a pro basketball player in Japan. Wife and I asked our daughter to send his earnings information so we could help them file income tax here in the US. When he sent the info it was a scan of the document and it was in Japanese and we are unable to translate. Could someone help with this please? See attached jpg. Thanks

The left column in the table are dates.
平成 is the name of the Heisei period, and is used to state the year. 月 means month. 1月 is January, 10月 is October, etc. |
