Could anybody please translate this short email for me to Japanese?

1.　Move the last word 敬具 to furthest right.
2.　Where you see the three squares in the first line, it should be just one letter ま.

There is another thing
The site I ordered is in Japanese only, although I am able to identify some kanji (I speak Chinese) there is still one problem.



They want me so select a subject but I can't translate them
Can somebody tell me which subject is the one I shall use?

Should be either 「出荷後のお届けについて」 or 「その他のお問い合わせ」.

In short, the problem is with your credit card. It didn't go through. Check with your credit card company if you want to re-order.
