No, those are different. 申込書 means 'application form' or 'order form'. 請求書 is a piece of paper asking you to pay for something, such as gas, electricity, etc.

弁当 (べんとう) = Boxed meal you bring to work, school, picnic, etc

給食 (きゅうしょく) = Hot lunch served at school
