In this case, りょうやく was written as れうやく in old style Japanese. Thus, this "良薬は口に苦し" is located here as start with れ.

The first child(sun) will be not smart (not keen?,).

The first child is often treated very carefully and kindly. Then he/she will make a person who is not keen to protect himself.

総領 means first sun.
甚六 means stupid first sun.
