I don't know how fugu is served in other countries, but in Japan, it's required by law that fugu be served without any poison. I've been eating fugu for a few decades and have never had a numb mouth. I just keep eating it because no other fish can give you the tender, creamy meat. I never let a winter pass me by without at least a couple of fugu dinners.

In the old days (over 40-50 yeras ago), however, things were a little different. The laws regarding fugu weren't nearly as strict as they are now. The so-called gourmets wanted to try a little further so that they could brag about their bravery later on. Now that is just forbidden.
It may be required by law to be served without poison, but the one time I did have it in Japan, my friend encouraged me to add a pinch of poison, as it numbs your tongue in a pleasurable way. I didn't oblige, but he did it. I understand the handful of fugu related deaths every year are usually due to this practice.
